Before Your Surgery Checklist

Pre-Registration

We ask that you complete the online preoperative registration form as soon as your surgery has been scheduled.  We need to receive your registration form prior to your procedure date.  If not received, the Pre-Admission Department will be contacting you prior to your date of service. In the presurgical registration form you will be given the option to receive preoperative texts pertaining to your appointment. If you chose not to receive texts, a Preadmission nurse will call you the day before your surgery, review special instructions and answer any questions you have.  If you have not received a call or text by 2 PM the business day before your surgery or have additional questions, please call us at 708-478-8889.

You may receive a financial phone call from a member of our Billing Team to discuss your Estimated Patient Responsibility (EPR) due on Date of Service (DOS).  Estimated Patient Responsibility may include your insurance deductible, co-insurance, and facility co-pay.  Remember that your bill from the facility is separate from the bills you will be receiving from your surgeon and your anesthesia provider.
